What is a Claim Form?

Required form to submit a paper BESTflexSM Plan or EBC HRASMclaim. Claims can also be submitted online using My Account Assistant or using our phone app, My Mobile Account Assistant

What is the BESTflex Plan Eligible Expense List?

A

A list of eligible expenses as well as expenses eligible with a doctor’s prescription.

What is the EBC HRA Eligible Expenses List?

A

A short list of eligible EBC HRA expenses. To view details of what's eligible for your EBC HRA plan design, please log into your account and review a copy of your My Company Plan by choosing "EBC HRA" from the menu.

What is the Direct Deposit Authorization Form?

A

Complete and return this form to us to have reimbursements from your BESTflex Plan or EBC HRA deposited directly into your checking or savings account. You can also authorize Direct Deposit by logging into My Account Assistant online and choosing "Activate Direct Deposit" from the menu."

What is the processing time for claims?

A

Our standard processing time is 1-2 business days from the receipt of your claim. If we have your e-mail address on file, we will notify you once your claim has been processed. Log into your account to check the status of your claim; your options are available from the menu under "Track.".

What is Participant Authorization Form?

A

Complete and return this form to us to authorize someone other than yourself with whom Employee Benefits Corporation can share your personal health information (PHI) on your behalf.
