Q

How do I submit a claim?

A

Once you incur an expense, you may file a Claim for reimbursement in one of three ways:

Log into My Account Assistant and select "Submit a New Claim" from the menu.

You can file a claim and attach documentation with My Mobile Account Assistant using your Android smartphone or Apple iPhone.

Complete an Employee Benefits Corporation Claim Form and attach itemized documentation. Then fax, e-mail, or mail it to us for processing:

To be approved, your claim must be for an eligible expense with a date of service within your Plan Year. The documentation you submit to verify your claim must include the date of service, the type of service provided, and the amount owed. We do not need to see that a payment has been made for the expense.

Remember, if you are submitting a claim to your EBC HRA, you may be required to submit an Explanation of Benefits from the insurance carrier.

When completing a Claim Form, what do I use for my Benefit Code?

A

Make one of the following selections:

[F]- Heath Care FSA for BESTflex Plan medical claims
[L]- Limited Health Care FSA for participants who have an HSA to submit Dental or Vision claims
[D]- Dependent Care FSA for BESTflex Plan daycare claims
[I]- Individually Billed Insurance Premiums for BESTflex Plan individual insurance premium claims
[H]- HRA for EBC HRA claims

For faster processing, file your claim online by logging into your online account and completing the online form. You’ll get confirmation right away that your claim has been received and is in process.

What are the timelines for submitting claims?

A

Because each plan is different, please log into My Account Assistant and select your plan name from the menu.

Keep in mind that your expenses must incur within your plan year plus 2 ½ month grace period (when applicable) to be eligible. If you terminate employment or lose eligibility in your plan at any time during the year, your termination date becomes the last day of your plan year and services must incur by that date.

What is the processing time for claims?

A

Our standard processing time is 1-2 business days from the receipt of your claim. If we have your e-mail address on file, we will notify you once your claim has been processed. Log into My Account Assistant and select "Upcoming Payments" from the menu.

Where can I view a list of eligible expenses?

A

For a list of eligible BESTflex Plan and EBC HRA expenses, please review the Eligible Expenses lists in the "Quick Forms" dropdown at www.ebcflex.com. For details on what's eligible for your EBC HRA plan design, please log into your account and review a copy of your My Company Plan by selecting "EBC HRA" from the menu..
